Chestnut Health Systems is seeking full-time Youth and Family
Support Services Coordinator to work at our Belleville, IL
facility, servicing the Metro East and St. Louis, MO areas.

use, mental health and primary care treatment to patients in

Supervise the Outpatient and School Based programs in St. Clair
County including the System of Care Grant.
Coordinate development, planning, implementation and evaluation of
services according to all governing policies and procedures to
improve services for youth and families.
Set an example for and reinforce evidence-based practices,
including but not limited to, the recovery philosophy utilizing the
elements of hope, respect, empowerment, health/wellness and
spirituality/connectedness, trauma informed care, harm
Act as a role model and provide leadership and direction to staff
to ensure positive interaction within Chestnut and the community to
sustain a favorable program reputation, especially related to
working directly with school districts, courts, and DCFS.
Train, ensure competencies and evaluate staff, interns and
Coordinate referrals and maintain referral sources.
Provide direct services to consumers and their families at an
assigned level of service expectation.
Promote Chestnut’s culture of customer service excellence through
the adherence to Chestnut’s behavioral standards for customer
Maintain and protect confidentiality and security of all
organizational information gained in the course of performing job
responsibilities including, but not limited to, financial and
Perform other duties as assigned, required or specifically
negotiated with supervisor.
Must have a Master’s degree in a human services field; Or a
Bachelor’s degree with at least five years of human service
Supervisory experience preferred.
Must have a valid driver’s license, private automobile insurance
and be insurable.
Requires basic computer skills and experience to include MS Word
and Excel, Outlook, and an electronic medical record (EMR).
