worker to elevate Senior Care - Strong referral base: Taps into the
large SSM referral network Role & Schedule - FTE: 1.0 - Days:
Monday-Friday - Outpatient: 100% - Call: Weekday self-call for ones
own patients What Youll Do - Provide comprehensive primary care
with a special emphasis on Senior Care, partnering closely with the
embedded care team to improve access, coordination, and outcomes -
Help establish and grow the clinics patient panel in a high-demand
