$315,000 base salary Bonus based on RVU production Quality bonus up
to 10% of base salary Benefits: Full Benefits $40,000 Sign-on bonus
$30,000 total retention bonus $15,000 in relocation reimbursement
33 days of PTO 5 days $6,000 CME annually Malpractice with tail
